How to Prevent It: What the First 48 Hours Require

The window in which the blood clot is most vulnerable is the first 24 to 48 hours following extraction. The guidelines during this period are specific and evidence-based:

Do not smoke. If there is ever a period in which quitting or pausing smoking has a direct and immediate clinical benefit, the 72 hours following an extraction is it. The risk is highest in this window, and the consequence of dislodging the clot is significant.

Do not use a straw. The suction created draws air and liquid through the mouth in a way that can disturb the socket.

Do not rinse forcefully or spit. The first 24 hours are not the time for vigorous mouthwash use. Gentle salt water rinses can begin after 24 hours — they are beneficial for keeping the socket clean — but they must be very gentle. Swish lightly and let the water fall out of the mouth rather than spitting.

Avoid hot liquids immediately after the procedure. Heat can cause vasodilation and may affect clot formation. Stick to lukewarm or cool food and drinks for the first several hours.

Eat soft foods on the opposite side. Avoid chewing anywhere near the extraction site for the first few days. Foods that require significant biting pressure can disturb the area.

Keep up gentle oral hygiene elsewhere in the mouth. Brush normally except directly around the socket. Good overall oral hygiene reduces bacterial load, which supports healing at the extraction site.

Recognising Dry Socket: Signs to Watch For

The key distinguishing features of dry socket are:

• Pain that worsens two to four days after the extraction, rather than improving progressively
• Pain that radiates beyond the extraction site — along the jaw, toward the ear, or up into the temple
• A visible empty or whitish-looking socket when you look in the mirror — in healthy healing, the socket should be dark with a visible clot
• A distinctive bad taste or smell coming specifically from the socket
• Swollen lymph nodes in the neck in some cases
